You have two options, you can complete a measurement yourself using an online app (not recommended) or during the prescription options screen, select Custom On-Site Measurements.

There's no need to worry about the cost of our premium service, we feel a professional touch is a requirement not an option and therefore should be affordable.  We offer premium delivery for free for orders over $150 (limited time!) and only <s>$49.99</s> $29.99 for any orders under that limit.
